Process

To legally change your name in Manitoba, you must complete the Manitoba application for an adult legal change of name. The fee is $100.

Requirements

Eligibility

The first step is to fill out a name change application form. With this form, you can only change your name if you have lived in Manitoba for the last three months, and meet the criteria for being an “adult”.

To be an “adult”, you have to meet one of the following criteria:

  • You’re over 18
  • You’re married, or in a common-law relationship
  • You’re a parent with custody of a child

If you’re unable to fill out a form, and you have a Committee or Substitute Decision Maker, you may have them fill it out.

Commissioning

Affidavits must be signed by a Commissioner for Oaths, RCMP, lawyer, notary, judge, or other authorized person (see The Evidence Act of Manitoba). Witnesses can also confirm photocopies of your documents by comparing originals and signing the copies.

If mailing, use a tracked service if possible. Fees can be paid by cash, debit, or credit in person.

If accepted, you will receive instructions for a certified criminal record check. Do not undergo a criminal record check until instructed.

Your change of name certificate will show both your previous and new names. Use this certificate to update other documents, like your driver's license, photo card, or health card.
